Put your situation into words the CRA will understand.

Most refused applications are not refused because the person does not qualify. They are refused because the form did not describe the effect of the impairment clearly enough. This worksheet helps you do that part properly, in your own words, before you fill in anything official.

There is no account, no upload, and nothing sent to us or anyone else. This page has no way to send your answers anywhere — everything happens inside your own browser. When you are finished you get your own text back to copy, and what you do with it is entirely your business.

How to use it

  • Only fill in the sections that actually affect you. Most people use two or three.
  • Be specific and unflattering. “It takes me twenty minutes to walk one block and I stop twice” is worth far more than “I have trouble walking”.
  • Describe a typical bad day, not your best day. The CRA is asking what your life is actually like.
  • Answer as things are even with your medication, therapy, and any devices you use. That is the test the CRA applies.
  • When you are done, press Build my notes at the bottom and copy the result.
This is not the CRA form

This worksheet produces notes for you. The official form is T2201, and it must be completed on the CRA’s own form or through their website. Your notes are the raw material to work from — and something your medical practitioner can read before they complete their section.

Which route might apply?

There are three ways to qualify. Tick anything that sounds like you — you do not have to be sure.

Two more things

Remember the twelve-month rule

The CRA only counts an impairment that has lasted, or is expected to last, at least 12 continuous months. If yours began recently but is expected to continue, say so in the box above.
